When I signed in I had to sign a waiver before the extraction of my tooth. I was told I would be given the info on the waiver before the extraction. I was never given any information on the waiver. I have no idea what I signed! The extraction went well and I was given post op instructions. The dentist and assistant were very kind and caring.
